To apply to this rotation please follow the steps listed below.
- Complete the form that applies to you with personal information - US citizen or foreign national.
- A current curriculum vita, which should include:
- Medical school - name, year entered, graduation date
- Residency (if applicable) - name of program, year entered, graduation date
- Undergraduate education - name of university, address, degree, major and graduation date
- Graduate education (if applicable) - name of graduate school, address, degree, major, and graduation date.
- Work and research experience
- Publications
- Community service
- Hobbies and interests
- An essay in the form of a letter, at least one page in length stating the reasons you want to participate in this
elective and what your future goals and career plans are in relation to the elective.
- An official copy of your medical school transcript.
- A letter of recommendation with a focus on describing your interpersonal skills.
- A letter from your University Dean, Program Director or Residency advisor stating that: you are in good
academic standing and have approved your elective.
This information must be received
- US citizens - at least 6 months prior to a selected rotation date.
- Foreign National's - at least 9 months prior date.

All materials submitted must be complete and accurate. Your personal information will be submitted to the security
office for background checks. If the information is not accurate your clearance may be denied. If any information
changes prior to the rotation you must notify of us as soon as possible.
Note: as a foreign national if you are accepted, you will then need to complete a full security background process.
After acceptance you will be sent detailed background forms and a fingerprint card. This packet of forms must be
completed in a timely manner.
